Why do I need to be a SNAP PCF member?
The Max Card is one of the opportunities SNAP PCF provides for local parent carers.
But there is an important part you can play too.
Your voice helps us keep opportunities like this available.
SNAP PCF uses parent carer participation and feedback to show funders and partners what matters to families and the difference our work makes.
If you are a SNAP PCF member, we ask you to get involved by completing our surveys when you can.

Why does this matter?
Your responses help SNAP PCF demonstrate the experiences and needs of Central Bedfordshire families when we are working with services, decision-makers and funders.
The more parent carers who take part, the stronger the evidence we have.
This evidence is important when we are seeking funding to continue and develop opportunities and support for local families.
So, by being a SNAP PCF member and taking part in our surveys, you are helping us make sure the parent carer voice is heard.
SNAP PCF Max Card Eligibility
Introduction
Thank you for applying for a Max Card through SNAP Parent Carer Forum (SNAP PCF).
To ensure we issue Max Cards to families who meet the eligibility criteria, we need to verify your child or young person's Special Educational Need and/or Disability (SEND). This short survey asks for some basic information and allows you to upload evidence to support your application.
What evidence can I provide?
Examples of evidence you can provide include:
An Education, Health and Care Plan (EHCP);
A Disability Living Allowance (DLA) award letter;
A Personal Independence Payment (PIP) award letter, where applicable;
A letter from the NHS or another health professional;
Evidence or a letter from your child's school or SENCO; or
Other appropriate evidence that confirms your child's SEND or disability.
Please only provide the evidence needed to confirm eligibility. You do not need to upload your child's complete EHCP, full medical records or information unrelated to their SEND or disability.
Where possible, please redact unnecessary information before uploading your document, including financial information, detailed medical information or information about other family members.
Please make sure your child's name and the information needed to confirm eligibility are visible.

Data Protection
Your information and evidence
SNAP PCF takes the protection of your personal information seriously.
The information and evidence you provide may include information about your child's health, disability or SEND and may therefore constitute special category personal data under UK GDPR.
SNAP PCF will use this information only to verify eligibility for a Max Card and will restrict access to authorised individuals who need to carry out this check.
We will collect and use only the information necessary for this purpose.
Once eligibility has been verified, we will normally delete the supporting document. We will retain a record that eligibility has been verified rather than retaining the full supporting document, unless there is a specific and documented reason why the evidence needs to be retained.

What happens next?
Once you submit your application, SNAP PCF will check the information and evidence provided to confirm eligibility.
If we need further information, we will contact you using the email address provided.
Once eligibility has been confirmed, your Max Card application will be processed.
If we are unable to confirm eligibility, we will explain why and, where appropriate, let you know what additional information may be needed.
Please note that it can take up to 4 weeks to process your application.


Terms & Conditions:
We currently give members a free MAX Card when they join our membership. If you would like a MAX Card, please email admin@snappcf.org.uk to request one, as they are NOT automatically sent out, they MUST be requested.
Max Card has negotiated with each organisation, and each organisation offers something different. SNAP PCF is not responsible for what eligibility or proof you may be asked to provide when booking.
The card is property of the child, so in order to obtain the discount the child must be present. In many cases the discount is extended to carers and accompanying family members, but this varies between the venues. Venues reserve the right to ask for proof of identity upon presentation of the card. Please ensure that before you visit the attraction you are certain about how many accompanying adults and children are allowed.

WHIPSNADE ZOO
Please be sure to familiarise yourself with the booking information.
Families can purchase up to 6 tickets with a Max card, costing £6 for adults and £3 for children. Tickets cannot be pre-booked and are only available on the day, to be purchased at the entrance.
